National Project Management of the Healthcare Communities Knowledge Gateway

The objectives of this collaborative agreement is to provide technical assistance to HRSA grantees and other providers of healthcare services by supporting the Healthcare Communities Knowledge Gateway, a robust knowledge and content management system portal, and related quality improvement efforts that
bridge healthcare, public health, local and state based community organizations in improvement efforts across the United States.

Healthcare Communities Knowledge Gateway provides support to the following: (a) Patient Safety and Clinical Pharmacy Services Collaborative (PSPC) participant organizations and related community based activities; (b) the Donation and Transplantation Community of Practice (DTCP) continued growth and success in saving lives and increasing awareness of the need for additional donors; and (c) information resources for the State Health Access program.

Uses and Use Restrictions

The cooperative agreement recipient will operate and manage the Healthcare Communities Knowledge Gateway main portal and resources center, results-sharing site, and help desk or create an equivalent system without disruption of service to the current users such as eligible organizations and a variety of HRSA grantees including but not limited to: health centers, Counties and States, transplant hospitals and centers, qualified organ procurement organizations, educational institutions, state and local health departments, public health and other private entities.

Eligibility Requirements

Applicant Eligibility

Public, private, for profit and not-for-profit entities are eligible to apply.

The applicant must have the ability to provide technical support and development; national project management of community based improvements, results sharing with automated analysis, and associated resources; and improve community resources including payer and health information technology systems.

Beneficiaries include current users such as eligible organizations and a variety of HRSA grantees including but not limited to: health centers, Counties and States, transplant hospitals and centers, qualified organ procurement organizations, educational institutions, state and local health departments, public health and other private entities.

This program is excluded from coverage under OMB Circular No. A-102. OMB Circular No. A-110 applies to this program. HRSA requires all applicants to apply electronically through Grants.gov.

Award Procedures

All qualified applications will be forwarded to an objective review committee. Based on the advice of the objective review committee, the HRSA program official with delegated authority is responsible for final selection and funding decisions. Notification is made in writing by a Notice of Award.

This cooperative agreement will provide funding for 3 years. Approximately $1,000,000 is expected to be available annually to fund one cooperative agreement. Applicants may request up to $1,000,000 per year. Funding beyond the first year is dependent on the availability of appropriated funds in subsequent fiscal years, awardee satisfactory performance, and a decision that continued funding is in the best interest of the Federal government.

Length and Time Phasing of Assistance

This cooperative agreement will provide funding for 3 years. Approximately $1,000,000 is expected to be available annually to fund one cooperative agreement. Applicants may request up to $1,000,000 per year. See the following for information on how assistance is awarded/released: Grantee drawdown funds, as necessary, from the Payment Management System (PMS). PMS is the centralized web based payment system for HHS awards.

Reports

The awardee must submit a progress report to HRSA on a quarterly basis.

Sample text describing the report: This report has two parts.

The first part demonstrates grantee progress on program-specific goals.

The second part collects core performance measurement data including performance measurement data to measure the progress and impact of the project.

Further information will be provided in the award notice.

Submit a quarterly electronic Federal Financial Report (FFR) Cash Transaction Report via the Payment Management System.

The report identifies cash expenditures against the authorized funds for the award.

The FFR Cash Transaction Reports must be filed within 30 days of the end of each quarter.

Failure to submit the report may result in the inability to access award funds.

Audits

In accordance with the provisions of OMB Circular No. A-133 (Revised, June 27, 2003), "Audits of States, Local Governments, and Non-Profit Organizations," nonfederal entities that expend financial assistance of $500,000 or more in Federal awards will have a single or a program-specific audit conducted for that year. Nonfederal entities that expend less than $500,000 a year in Federal awards are exempt from Federal audit requirements for that year, except as noted in Circular No. A-133.

Records

Grantees are required to maintain grant accounting records 3 years after the date they submit the FFR. If any litigation, claim, negotiation, audit or other action involving the award has been started before the expiration of the 3-year period, the records shall be retained until completion of the action and resolution of all issues which arise from it, or until the end of the regular 3-year period, whichever is later.

This program is subject to the provisions of 45 CFR Part 92 for State, local and tribal governments and 45 CFR Part 74 for institutions of higher education, hospitals, other nonprofit organizations and commercial organizations, as applicable.
