Affordable Care Act (ACA) Nurse-Managed Health Clinics

To provide federal funding to support the development and operation of Nurse-Managed Health Clinics (NMHC) to: 1) improve access to primary health care, disease prevention and health promotion in medically underserved areas (including enhancements of outreach strategies); 2) enhance nursing practice
by increasing the number of structured clinical teaching sites for undergraduate and graduate nursing students; and 3) enhance electronic processes for establishing effective patient and workforce data collection systems.

Under this program, the focus would support the training and practice development site for nurse practitioners to build the capacity of primary care provider workforce.

Uses and Use Restrictions

Awards may be used to support the operation of nurse managed centers, implement new technologies, training programs for nurse practitioner students and other students of other health related disciplines, and enhancement of patient care and accessibility.

Eligibility Requirements

Applicant Eligibility

Eligible applicants must be nurse-practice arrangements managed by advanced practice nurses, that provide primary care or wellness services to underserved or vulnerable populations and is associated with a school, college, university, or department of nursing, federally qualified health center or independent nonprofit health or social services agency.

Beneficiary Eligibility

Beneficiaries include the following: individual/family; specialized group; health professionals; education profession; student/trainee; graduate student.

Credentials/Documentation

Applicants should review the individual HRSA funding opportunity announcement issued under this CFDA program for any required proof or certifications which must be submitted prior to or simultaneous with submission of an application package.

Any Nurse Managed Clinic that will have undergraduate or graduate nursing students receiving clinical education or experiences during this grant period MUST provide a Memorandum of Agreement with the school(s) of nursing for clinical placement arrangement. This program is excluded from coverage under OMB Circular No. A-87.

All qualified applications will be forwarded to an objective review committee. Based on the advice of the objective review committee, the HRSA program official with delegated authority is responsible for final selection and funding decisions.

This report entitled, "BHPr Progress Report," has two parts.

The first part demonstrates grantee progress on program-specific goals.

The second part collects core performance measurement data to measure the Bureau s progress through its grantees in: (1) improving the distribution, diversity, and quality of the healthcare workforce, (2) improving the educational environment infrastructure, and (3) increasing students selection of primary care education.

Progress reports are generally due in August each year and must be submitted on-line by grantees in the Electronic Handbooks system at https://grants.hrsa.gov/webexternal/home.asp.

All BHPr grantees are required to submit a final report within 90 days after the project period ends.

The final report collects program-specific goals, core performance measurement data, grantee objectives and accomplishments, barriers encountered, and responses to summary questions regarding the grantee s overall experiences over the entire project period.

The final report must be submitted on-line by grantees in the Electronic Handbooks system at https://grants.hrsa.gov/webexternal/home.asp.

Submit a quarterly electronic Federal Financial Report (FFR) Cash Transaction Report via the Payment Management System.

The report identifies cash expenditures against the authorized funds for the grant.

The FFR Cash Transaction Reports must be filed within 30 days of the end of each quarter.

Failure to submit the report may result in the inability to access award funds.

Audits

In accordance with the provisions of OMB Circular No. A-133 (Revised, June 27, 2003), "Audits of States, Local Governments, and Non-Profit Organizations," nonfederal entities that expend financial assistance of $500,000 or more in Federal awards will have a single or a program-specific audit conducted for that year. Nonfederal entities that expend less than $500,000 a year in Federal awards are exempt from Federal audit requirements for that year, except as noted in Circular No. A-133.

Records

Grantees are required to maintain grant accounting records 3 years after the date they submit the Federal Financial Report (FFR). If any litigation, claim, negotiation, audit or other action involving the award has been started before the expiration of the 3-year period, the records shall be retained until completion of the action and resolution of all issues which arise from it, or until the end of the regular 3-year period, whichever is later.

Criteria for Selecting Proposals

Approval of an application will be based on analysis of the factors set forth in the applicant s proposal. See the section on Application Review in the funding opportunity announcement. The program has a funding preference shall be granted to applicants with projects that will substantially benefit rural or underserved populations, or help meet public health nursing needs in State or local health departments.
