Uses and Use Restrictions
The cooperative agreement recipient will engage HRSA and DHHS grantees in a variety of activities that will build capacity at the community level to transfer knowledge, skills, and practical approaches to quality management around the prevention and treatment of overweight and obesity for children and families.
The targeted audience for this program includes HRSA and DHHS grantees and is inclusive of safety net providers, health departments, community-based organizations, and other constituents and stakeholders in the HRSA and DHHS Program network.
Funds may not be used to support the following: the delivery of health care services, Construction, or Alteration or renovation.

All qualified applications will be forwarded to an objective review committee. Based on the advice of the objective review committee, the HRSA program official with delegated authority is responsible for final selection and funding decisions. Notification is made in writing by a Notice of Award.

Status Reports:
1) Submit a Financial Report.
A financial report is required within 90 days of the end of each budget period.
It is submitted electronically through the HRSA EHBs.
The report is an accounting of expenditures under the project that year.

2) Submit a Progress Report(s).

3) Submit a Final Report.
A final report is due within 90 days after the project period ends.
The final report collects program-specific goals, core performance measurement data, awardee objectives and accomplishments, barriers encountered, and responses to summary questions regarding the awardee s overall experiences over the entire project period.
The final report must be submitted on-line by awardees in the Electronic Handbooks system at https://grants.hrsa.gov/webexternal/home.asp.
Submit a quarterly electronic Federal Financial Report (FFR) Cash Transaction Report via the Payment Management System.
The report identifies cash expenditures against the authorized funds for the cooperative agreement.
The FFR Cash Transaction Reports must be filed within 30 days of the end of the quarter.
Failure to submit the report may result in the inability to access award funds.

Audits
In accordance with the provisions of OMB Circular No. A-133 (Revised, June 27, 2003), "Audits of States, Local Governments, and Non-Profit Organizations," nonfederal entities that expend financial assistance of $500,000 or more in Federal awards will have a single or a program-specific audit conducted for that year. Nonfederal entities that expend less than $500,000 a year in Federal awards are exempt from Federal audit requirements for that year, except as noted in Circular No. A-133.
Records
Award recipients are required to maintain cooperative agreement accounting records 3 years after the date they submit the final Federal Financial Report. If any litigation, claim, negotiation, audit or other action involving the award has been started before the expiration of the 3-year period, the records shall be retained until completion of the action and resolution of all issues which arise from it, or until the end of the regular 3-year period, whichever is later.

Criteria for Selecting Proposals
