Prevention Public Health Fund 2012: Viral Hepatitis Education

Program activities under this funding will allow CDC to partner with organizations to expand and improve efforts to develop and disseminate viral hepatitis training and educational materials targeting the public and private sector health care professionals to build capacity to assess, test and medically
manage populations at risk for chronic HBV and HCV infection.

Uses and Use Restrictions

Funds may be used for costs associated with establishing and maintaining a prevention and control program directed towards prevention of viral hepatitis infections and its manifestations.

Recipients may only expend funds for reasonable policies, systems and program purposes including personnel, travel, supplies and services, such as reducing transmission of HBV and HCV, preventing and delaying the progress of chronic infection, as well as education and training of the general public and health care providers.

Recipients may not use funds for clinical care.

Recipients may not generally use funding for the purchase of furniture or equipment.

Any such proposed spending must be identified in the budget.

The recipient must perform a substantial role in carrying out project objectives and not merely serve as a conduit for an award to another party or provider who is eligible.

Applicants include State or local governments or their Bona Fide Agents, public and private nonprofit organizations, for profit organizations, small, minority, women-owned businesses, universities, colleges, research institutions, hospitals, community-based organizations, faith based organizations, Federally recognized Indian tribal governments, Indian tribes, and Indian tribal organizations.

Application Procedures

OMB Circular No. A-102 applies to this program. OMB Circular No. A-110 applies to this program. Applicants must download application forms from www.Grants.gov. Award Procedures

Successful applicants will receive a Notice of Award (NOA) from the CDC Procurement and Grants Office. The NOA shall be the only binding, authorizing document between the recipient and CDC. The NOA will be signed by an authorized Grants Management Officer and emailed to the program director and a hard copy mailed to the recipient fiscal officer identified in the application.

Sections 301(a) and 317N of the Public Health Service Act (42 U.S.C. Section 241(a) and 247b-15) as amended.

Range of Approval/Disapproval Time

Renewals will be based upon the conditions in the funding announcement and are subject to the availability of funds under the Patient Protection and Affordable Care Act.

Post Assistance Requirements

Reports

Program reports are not applicable.

Cash reports are not applicable.

Interim and annual reports will be required as outlined in the Funding Opportunity Announcement (FOA).

The reporting schedule will be based on the requirements listed in the FOA.

Where applicable, the interim progress report is due no less than 90 days before the end of the specified budget period.

Where applicable, the Interim Progress Report will serve as the non-competing continuation application.

Federal Financial Reports (FFRs) (SF425) are required no more than 90 days after the end of the budget period.

Final FFR and progress reports are due no later than 90 days after the end of the project period.

Performance monitoring is not applicable.

Audits

In accordance with the provisions of OMB Circular No. A-133 (Revised, June 27, 2003), "Audits of States, Local Governments, and Non-Profit Organizations," nonfederal entities that expend financial assistance of $500,000 or more in Federal awards will have a single or a program-specific audit conducted for that year. Nonfederal entities that expend less than $500,000 a year in Federal awards are exempt from Federal audit requirements for that year, except as noted in Circular No. A-133. Records must be available for review or audit by appropriate officials of the Federal agency, pass-through entity, and General Accounting Office (GAO). The grantee is to also ensure that the sub-recipients receiving CDC funds also meet these requirements (if total Federal grant or grants funds received exceed $500,000). The grantee should include this requirement in all sub-recipient contracts.

Records

Financial records, supporting documents, statistical records, and all other reports pertinent to the program shall be retained for a minimum of 3 years, or until completion and resolution of any audit in process or pending resolution. In all cases records must be retained until resolution of any audit questions. Property records must be retained in accordance with 45 CFR 92.42.

Criteria for Selecting Proposals

Applications will be reviewed for completeness by the Procurement and Grants Office (PGO) staff and for responsiveness jointly by Division of Viral Hepatitis (DVH), NCHHSTP. Incomplete applications and applications that are non-responsive to the eligibility criteria will not advance through the review process. Applicant will be notified that the application did not meet submission requirements.
CDC will conduct a review to evaluate complete and responsive applications according to the criteria listed in Section V. Application and Submission information, subsection "Evaluation Criteria" within the individual funding opportunity announcement.
