The purpose of this notice of funding opportunity (NOFO) is to build on the National Center for Chronic Disease Prevention and Health Promotion’s (NCCDPHP) efforts to strengthen the capacity and capabilities

of the states’ chronic disease prevention and health promotion public health workforce.
This program is expected to result in adoption of evidence-based chronic disease prevention and health promotion approaches and effective use of public health tools and resources.
This 5-year NOFO supports strengthening the leadership and expertise in the chronic disease prevention system domains in order to transform the practice of chronic disease prevention and health promotion in state health departments and accelerate improvements in population health.
Focusing on the priorities and goals of the NCCDPHP, this program will support the development of effective state chronic disease programs through implementation of practice-based learning and leadership development strategies.
Recipients will include one (1) organization that will address these strategies and activities:
Strategy:
Practice-based Learning Activity 1:
Convene and lead peer-to-peer forums that engage in problem solving and learning about effective domain and workforce development strategies.
Activity 2:
Assess state health department organizational capacity and develop an action plan.
Activity 3:
Expand use of technology by developing learning opportunities in modes most accessible to state health department staff.
Strategy:
Leadership and Development Activity 1:
Develop an implementation plan that describes activities to build leadership skills and competencies among Chronic Disease Directors and staff.
Activity 2:
Expand workforce training to build identified leadership skills and competencies.
Activity 3:
Develop leadership development opportunities.
Activity 4:
Model, translate, and disseminate successful leadership approaches.
Activity 5:
Evaluate impact.
This NOFO will include evaluation to ensure continuous program quality improvement, document successful learning and leadership development approaches, and to inform decisions about future workforce learning and development strategies.
