Affordable Care Act (ACA) Family to Family Health Information Centers

To carry out special maternal and child health (MCH) projects of regional and national significance for the development and support of Family to Family Health Information Centers.

The Family to Family Health Information Centers (A) assist families of children with disabilities or special health
care needs to make informed choices about health care in order to promote good treatment decisions, cost-effectiveness, and improved health outcomes for such children; (B) provide information regarding the health care needs of, and resources available for, such children; (C) identify successful health delivery models for such children; (D) develop with representatives of health care providers, managed care organizations, health care purchasers, and appropriate State agencies, a model for collaboration between families of such children and health professionals; (E) provide training and guidance regarding caring for such children; (F) conduct outreach activities to the families of such children, health professionals, schools, and other appropriate entities and individuals; and (G) are staffed by such families who have expertise in Federal and State public and private health care systems; and by health professionals.

Program Accomplishments

Fiscal Year 2009: 51 projects funded for activities supported under this CFDA. Fiscal Year 2010: 51 projects funded for activities supported under this CFDA. Fiscal Year 2011: 51 projects funded for activities supported under this CFDA.

Uses and Use Restrictions

All funds granted should be expended solely for carrying out the approved project in accordance with Section 501(c)(A) of the Social Security Act.

Eligibility Requirements

Applicant Eligibility

Eligible applicants for the Family to Family Health Information Centers program include any public or private entity, faith-based or community-based organization that can document previous experience to address and fulfill review criteria in the guidance.

Only organizations that are staffed by families in the 50 States and the District of Columbia are eligible.

Applicants must have experience related to the "purpose" of the program, must have a focus on health for children and youth with special health care needs (CYSHCN) (as defined by the MCH Bureau), and the effort must be family-run.

If an applicant is a family organization with a history of funding that is condition specific or related to education, mental health or developmental disabilities, as with any applicant, evidence of health for the broad CYSHCN population must be evident in their application.

If a different fiduciary agency is needed, such as a university, activities must still be family-run for the entire grant cycle.

Beneficiary Eligibility

Projects will benefit (1) public or private agencies, organizations and institutions engaged in activities for CYSHCN; (2) family members and children who receive services through the program; and (3) professionals and trainees who provide services to CYSHCN.

Applicants should review the individual HRSA funding opportunity announcement issued under this CFDA program for any required proof or certifications which must be submitted prior to or simultaneous with submission of an application package. OMB Circular No. A-87 applies to this program.

Award Procedures

All qualified applications will be forwarded to an objective review committee. Based on the advice of the objective review committee, the HRSA program official with delegated authority is responsible for final selection and funding decisions.
Notification is made in writing by a Notice of Award.

Authorization

Social Security Act, Title V, Section 501(c)(1)(A)(iii), (42 USC 702) as amended; the Patient Protection and Affordable Care Act of 2010, Section 5507 (P.L. 111-148).

Renewals

After initial award, projects may be renewed annually up to the limit of the project period upon the submission and approval of a satisfactory continuation application.

Length and Time Phasing of Assistance

Competing and competing continuation grant applications are submitted for three (3) years of support. See the following for information on how assistance is awarded/released: Grantees drawdown funds, as necessary, from the Payment Management System (PMS). PMS is the centralized web based payment system for HHS awards.

Post Assistance Requirements

Reports

Annual program reports, annual program service reports, and special reports as required.

Submit a quarterly electronic Federal Financial Report (FFR) Cash Transaction Report via the Payment Management System.

The report identifies cash expenditures against the authorized funds for the grant.

The FFR Cash Transaction Reports must be filed within 30 days of the end of each quarter.

Failure to submit the report may result in the inability to access grant funds.

Annual financial reports and annual narrative progress reports.

Submit a Financial Status Report (FSR or SF-269) directly to HRSA.

The report identifies cash expenditures against the authorized funds for the grant.

The Financial Status Report must be filed semi-annually or annually depending on the terms and conditions of award.

Failure to submit the report may result in the inability to access grant funds.

No performance monitoring is required.

Audits

In accordance with the provisions of OMB Circular No. A-133 (Revised, June 27, 2003), "Audits of States, Local Governments, and Non-Profit Organizations," nonfederal entities that expend financial assistance of $500,000 or more in Federal awards will have a single or a program-specific audit conducted for that year. Nonfederal entities that expend less than $500,000 a year in Federal awards are exempt from Federal audit requirements for that year, except as noted in Circular No. A-133.

Records

Grantees are required to maintain grant accounting records 3 years after the date they submit the Federal Financial Report (FFR). If any litigation, claim, negotiation, audit or other action involving the award has been started before the expiration of the 3-year period, the records shall be retained until completion of the action and resolution of all issues which arise from it, or until the end of the regular 3-year period, whichever is later.

This program is subject to the provisions of 45 CFR Part 92 for State, local and tribal governments or 45 CFR Part 74 for institutions of higher education, hospitals, other nonprofit organizations and commercial organizations, as applicable.
