The Centers for Disease Control and Prevention (CDC) announces the availability of funds to develop multisector action plans that address the social determinants of health (SDOH) by accelerating action in state, local, territorial, and tribal jurisdictions that lead to improved chronic health conditions


among Americans experiencing health disparities and inequality.

Eligibility is limited to State Governments, County Governments, City ot Township Governments, Special District Governments, Native American tribal governments (Federally recognized), Native American tribal organizations (other than Federally recognized tribal governments), State governments or their bona fide agents (includes the District of Columbia), Local governments or their bona fide agents, Territorial governments or their bona fide agents in the Commonwealth of Puerto Rico, the Virgin Islands, the Commonwealth of the Northern Marianna Islands, American Samoa, Guam, the Federated States of Micronesia, the Republic of the Marshall Islands, and the Republic of Palau, American Indian or Alaska Native tribal governments (federally recognized or state-recognized) and American Indian or Alaska Native tribal governments (federally recognized or state-recognized) as authorized by the Division H-Departments of Labor, Health and Human Services, and Education, and Related Agencies Appropriations Act, 2021, Joint Explanatory Statement, includes funding to establish the social determinants of Health pilot program as directed in House Report 116-450.

House Report 116-45, pages 76-77 provides for CDC to establish a Social Determinants of Health pilot program to award competitive grants to State, local, Territorial, or Tribal jurisdictions to support the development of Social Determinants of Health Accelerator Plans.

Applicants must be physically located and operate in the state, tribe, locality, or territory for which work is proposed.

Applicants must provide a statement that identifies the state or territory in which the applicant will do the proposed work.

This documentation must be labeled “Proposed_Work_Statement” and uploaded as a PDF file at www.grants.gov.

Applicants proposing to work in a state, tribe, locality, or territory other than where the applicant is physically located and operates will be considered non-responsive and will not receive further review.

The award ceiling for this NOFO is $125,000.

CDC will consider any application requesting an award higher than this amount as non-responsive and it will receive no further review.
