PPHF-2015-Empowering Older Adults and Adults with Disabilities through Chronic Disease Self-Management Education Programs Financed Solely by 2015 Prevention and Public Health Funds (PPHF-2015)

This funding opportunity is designed to bring to scale and sustain evidence-based self-management education programs that empower older adults and adults with disabilities from underserved areas and populations to better manage their chronic conditions.

The grants are intended to increase the

credit:


number of chronic disease self-management education program participants, while concurrently increasing the sustainability of these proven programs through innovative funding arrangements and by embedding the programs into the nation’s health and long-term services and supports systems.

The awards will be made in the form of cooperative agreements because AoA will be substantially involved.

Additional Information of Eligibility:
Eligible applicants are domestic public or private non-profit entities including state and local governments, Indian tribal governments and organizations (American Indian/Alaskan Native/Native American), faith-based organizations, community-based organizations, hospitals, and institutions of higher education.

Current 2012 PPHF CDSME grantees are not excluded from this funding opportunity but must provide a strong rationale for the need for additional funding.

Individuals, foreign entities, and sole proprietorship organizations are not eligible to compete for, or receive, awards made under this announcement.
