Promoting the Integration of Primary and Behavioral Health Care: Collaborative Care Model

The purpose of this program is to support implementation of the Collaborative Care Model (CoCM).

The CoCM is an evidence-based, integrated care approach that addresses mental and substance use conditions in primary care settings.

Care is provided by a primary care team and includes a case

credit:


manager, consulting psychiatrist, and other mental health professionals.

Recipients will be expected to work with at least three primary care practices to develop the staffing and systems necessary to implement the CoCM.

With this program, SAMHSA aims to increase the identification and treatment of mental health conditions for individuals who access care through primary care practices that too often go untreated and cause disability.

Additional Information of Eligibility:
Eligibility for this program is statutorily limited to a state or appropriate state agency.

Appropriate state agencies include the State Mental Health Authority, the Single State Agency (SSA) for substance use services, the State Medicaid agency, or the State Health Department.

State agencies participating in this program will work with at least three primary care practices to develop the staffing and systems necessary to implement the CoCM.
